In this presentation, Dr. Schnipper will discuss the importance of medication safety during care transitions, review the results of the first and second Multi-center Medication Reconciliation Quality Improvement Studies (MARQUIS1 and MARQUIS2), discuss ways to prevent adverse drug events during care transitions, and conclude with concrete next steps for ordering providers, pharmacists, nurses, and hospital and health system leadership.
