Please join KHA and the Cabinet for Health and Family Services on August 16, 2022 from 3:00 – 4:00 p.m. ET to discuss the Social Determinant of Health (SDOH) HRIP metric for year one and how you can meet the metric with the Kynect screening and resource tool.  Tracy Williams with the Cabinet for Health and Family Services will demonstrate the Kynect SDOH screening tool and the resources available through the on-line tool. 

Objectives:

• Define the hospital rate improvement program’s SDOH measure for calendar year 2022.
• Describe the Kynect on-line screening and resource tool for hospitals.

This is for all hospitals in KY including the Medical Center Bowling Green and the hospitals associated with the Medical Center as they are a part of the HRIP program.
